Do you struggle to just get through each day?

Do you often feel empty, sad or angry?

Depression Assessment

People who have been identified as depressed manifest a number of these symptoms below:

1. Do you frequently feel heavy, sad, and/or hopeless?

2. Do you often feel numb, empty, lacking feelings?

3. Do you lack energy to do the things you want to do?

4. Do you feel tired, helpless or worthless much of the time?

5. Do you have difficulty sleeping or sleep too much?

6. Have you lost interest in things you used to enjoy?

7. Do you often feel restless, irritable, and/ or angry?

8. Do you have difficulty concentrating or making decisions?

9. Do you participate in unsafe behavior such as excessive drinking, drug use, unsafe sex, taking risks in driving or other activities?

10. Do you ever feel that life just isn’t worth living and/or have thoughts of harming yourself or others?
